New VA study to test psilocybin for depression and PTSD
The Department of Veterans Affairs is launching a major clinical trial to test whether psilocybin, the active compound in magic mushrooms, can effectively treat depression and post-traumatic stress disorder in veterans. The study, called the Psilocybin Intervention for Veterans Overcoming Treatment-Resistant Depression or PIVOT, will enroll at least 240 veterans across five VA medical centers and run through June 2031. VA Secretary Doug Collins announced the initiative as part of the department's effort to evaluate new treatment options for veterans whose mental health conditions have not responded to currently available therapies.
This research comes at a critical time for veterans struggling with treatment-resistant mental health conditions. Major depressive disorder and PTSD remain among the most common and debilitating conditions affecting veterans, often persisting despite multiple rounds of medication and therapy. The VA currently offers evidence-based treatments including various antidepressants, cognitive behavioral therapy, and other psychotherapy approaches, but a significant portion of veterans continue to experience severe symptoms even after trying multiple treatment options. This trial represents the VA's recognition that new approaches are needed for those who have exhausted conventional treatments without finding relief.
The study will specifically enroll veterans diagnosed with major depressive disorder who have not responded to current treatments. Some participants will also have PTSD diagnoses, allowing researchers to examine psilocybin's effectiveness across both conditions. Veterans will be treated at five locations: the Birmingham VA Health Care System and Tuscaloosa VA Medical Center in Alabama, the VA Portland Health Care System in Oregon, Cpl. Michael Crescenz VA Medical Center in Philadelphia, and VA Puget Sound Health Care System in Seattle. The trial is open to veterans at these facilities who meet the clinical criteria for treatment-resistant depression.
The PIVOT study will use a double-blind, randomized design where veterans receive either psilocybin or a placebo along with therapeutic support. Participants will receive two doses spaced one month apart, with evaluation sessions two to four weeks after each dose and a follow-up six months after participation. The VA emphasized that all treatments will use pharmaceutical-grade drugs administered in safe, controlled clinical settings. This approach builds on preliminary research from Ohio State University, where a small 12-person study found that 75 percent of veterans with severe PTSD no longer met clinical criteria for the disorder after receiving synthetic psilocybin with psychotherapy, with no serious adverse effects reported.
This trial reflects broader federal momentum on psychedelic research following an executive order signed in April directing accelerated review of psychedelic therapies for mental health conditions. The VA is currently involved in 20 clinical trials examining psychedelic treatments including MDMA and LSD, though the FDA rejected an MDMA application for PTSD treatment in 2024, citing the need for additional safety and efficacy research. For disabled veterans, this represents a potential future treatment option, though it remains years away from general availability and depends entirely on trial outcomes demonstrating both safety and effectiveness.
Veterans interested in participating should contact the VA medical centers hosting the trial to inquire about eligibility screening. However, VA officials strongly cautioned against self-medication with psychedelics or replacing current mental health treatments with unprescribed substances. Veterans currently struggling with depression or PTSD should continue working with their VA mental health providers and explore the proven, evidence-based treatments already available at VA facilities.
